Schupan 2026 Wellness Program Overview
Welcome to the 2026 Wellness Program!
Employees and spouses enrolled in the Schupan Medical Plan can earn medical premium savings for 2027 by completing the 2026 Wellness Program requirements.
Earn Your Incentive
Medically enrolled employees can earn up to $1,000 in medical premium savings for 2027 by completing the required wellness activities.
Submission Deadline: November 30, 2026
(All required forms and documentation must be submitted by this date.)
Program Requirements & Incentives
To qualify for premium savings, the following requirements must be completed and submitted no later than November 30, 2026. Premium savings will be applied during the 2027 plan year.

New Hire Information
- New hires have 90 days from their date of hire to complete wellness requirements for the 2026 plan year.
- Wellness requirements completed for 2026 will also count toward the 2027 program.
- Premium savings are prorated annually and will be applied to the 2027 plan year as stated.

Medical Premium Savings & Forms
Employees and spouses enrolled in Schupan’s medical plan can earn Medical Premium Savings in 2027 by completing the Wellness Program.
If you are enrolled in the Schupan Medical Plan, complete the steps listed below to save up to $1,000 on your Medical Premiums in 2027.
Submission Deadline November 30, 2026
- Upload your completed Annual Physical (Proof of Visit Form)
- Upload your completed Preventive Screening Form.
- Upload your completed Biometrics Screening (Physician’s Results Form).

Enrolled Spouse:
- Submit one Annual Physical (Proof of Visit Form)
PLEASE NOTE:
- Kalamazoo area employees – Visit Health Centered (employee health center) to complete your biometrics exam and fill out the Physician Results Form. Appointments can be scheduled by calling (269) 924-0500.
- All other employees – email TMR@schupan.com to receive a voucher to visit LabCorp to complete your biometrics exam and fill out the Physician Results Form.
