1. Download the HealthJoy App.
2. Complete the Know Your Number Risk Assessment (KYN).

Employees Enrolled in Schupan’s Medical Plan

If you are enrolled Schupan’s Medical Plan, you can also save up to $1,000 on your Medical Premiums:

1. Complete steps 1 & 2 above.
2. Complete biometrics by November 30 & upload the Physician Results Form to the portal.
3. Complete & upload one (1) Preventive Screening Form (medical, dental or vision).
4. Complete one (1) e-Learning series.

Enrolled Spouse:

Submit one (1) Preventive Screening Form (medical, dental or vision).

How to log in:

Once downloaded, open your HealthJoy app and check your wallet for the Wellworks tab. Inside you will find instructions on how to log in.
